Important Update: New Patient Portal Coming Soon for Swain Community Hospital
Swain Community Hospital is launching a new patient portal, powered by InteliChart. This resource will give patients access to their medical records, beginning April 21, 2026.
What You Need to Know
- Our current patient portal, managed by Syntellis, will be sunsetting on April 30, 2026.
- Medical records from the current portal will not transfer to the new patient portal.
- Patients who wish to keep copies of their existing medical information should download their records before April 30, 2026.
What Action Should I Take?
We strongly encourage all patients who currently use the patient portal to:
- Log in to the current portal before April 30, 2026.
- Download or save your personal medical records for your records by following the steps below.
After April 30, 2026, your information will no longer be accessible through the current portal.
How to Download Records
- Log in to https://communicator.syntellis.com.
- Once logged into the patient portal, access the health record you want to download by clicking the "DOWNLOAD MY DATA" icon from the home screen.
- Next, click "Download Health Data."
- Under the "Download Health Data" section, select PDF or XML for the download format. Click the "BlueButton Download my data" icon to generate the CCD document. Note: If required by your browser, click "Yes" to view the exported file.
Note: Any patient data that you download from the portal is marked as "sensitive." This indicates that the file contains protected health information (PHI).
Need Records After April 30?
If you do not download your records before the portal sunsets, you may request copies of your medical records through our third-party medical records administrator, Verisma.

The hospital patient portal, offered by Swain Community Hospital, is a convenient, secure health management tool you can use anywhere you have access to the Internet. It provides access to important information about you and your visit(s) to our hospital.
Information available on the Swain Community Hospital Patient Portal includes:
- Admit and discharge dates and locations
- Procedures performed during admission
- Current and past problem list
- Current medication list and medication history
- Current medication allergy list and allergy history
- Immunization records
- Vital signs at discharge
- Laboratory test results (available at time of discharge)
- Demographics maintained by hospital (sex, race, ethnicity, date of birth, preferred language)
- Family & social history
Swain Community Hospital has partnered with Syntellis to provide this service. Online healthcare information is available to patients 18 years and older and who have been a patient at the hospital on or after January 1, 2014.
All Discharged Patients who supplied a valid email address will receive an electronic invitation from the hospital and a link to register online for Syntellis shortly after discharge.

WHAT IS THE PATIENT PORTAL?
The patient portal, offered by Harris Regional Hospital, is a convenient, secure health management tool you can use anywhere you have access to the Internet. It provides access to important information about you and your visit(s) to our hospital. Please note: If you add any information to your personal health record, we cannot access those changes. All questions regarding test results should be directed to the patients primary care or ordering physician.
FREQUENTLY ASKED QUESTIONS
Is the Patient Portal safe?
Yes, secure-socket layer encryption technology is utilized. When will health information/results be available on the Patient Portal? All information will be available within 36 hours of discharge. In most cases, information is available immediately following discharge.
Will historical health information be available on the Patient Portal?
Data will be available for visits beginning July 1, 2014, for those who choose to use the service. Data will not be available for visits registered prior to July 1.
Can patients print their health information from the Patient Portal?
Yes. Patients can download or print their personal results, or other health information.
I am locked out of my account, now what?
